Certification Packet

Non-Credit Student Limited Practice Certification 
Pursuant to Arizona Supreme Court Rule 38(d)
 

 Instructions 

After completing the following documents, deliver originals to:

Zelda Graham, Program Administrator
Sandra Day O'Connor College of Law
1100 South McAllister, Room 106
Tempe, AZ 85281 
 

• Copy of Unofficial Law School Transcript
• Sandra Day O'Connor College of Law Information Form (required for Dean Certification) - fill out, sign and date. 
• Arizona Supreme Court Application for Student Limited Practice Certification
  • Certification of Law Student- fill out, sign and date
  • Certification of Supervising Attorney - have supervising attorney fill out, sign and date (leave certification period blank)
  • Certification of Law School Dean - leave blank 

Once the Associate Dean has signed the Certification of Law School Dean, the Application will be returned to you.  Take the original and 2 copies to be filed with the Clerk of the Supreme Court at:

The Clerk will keep the original and stamp two copies.  Keep one copy and provide one copy to your supervising attorney.
